TPO pinpoints the leakage points where patients fall out of the care pathway before they get best-practice care. If you can show that your activities are closing those gaps—backed by measurable data—you can defensibly claim patient impact, not just a productive team.

IN PRACTICE

On one launch pathway, only 40% of eligible patients were getting the guideline-recommended screening test—the team’s TPO. They set a 45% 12-month target and tracked it. At 6 months, it had slipped to 39%—an early warning to course-correct—and by 12 months it hit 45%. The result: targeted activities, tied to a number that proves patients moved.
